Paperwork

Below is a list of all of the paperwork required for your child to attend YMCA Camp Minikani. Please note we do not cover camper health and medical expenses. All paperwork, unless noted is due by June 1, 2010.

Please note that we require a new, signed physical form once every 24 months. The remainder of the medical form should be updated annually. If you have a returning camper and are unsure of the date of the last medical examination that we have on file, or have any other questions please contact our camp office and we'll be more than happy to answer any questions.
